I have shared the form with my support group, and data is starting to come in. There's not enough responses so far (14) to do advanced breakdowns, but I have done a breakdown of what happened (COVID vaccination, other vaccination, reinfection...). I don't have the mental power yet to make it pretty - the X axis (not sorted!) is the change in symptom level, negative being worse, so -2 means a change from "Mild" to "Severe", while +2 is a change from "Severe" to "Mild" or from "Mild"/Medium" to "Gone. So far, noone started out with severe symptoms.
In hindsight, I should have added another question about how big the change was in the end. A week may not be enough for the immediate effects to die down, and there is room for noticeable change within each severity category.

| Effect of getting a COVID vaccination. Two cases of some improvement, one case of severe worsening |
 |
| Effect of getting a mild reinfection. Two cases of severe worsening, though they may be due to the immediate effects not being over yet. One case of big improvement |
 |
| In three cases, the long COVID symptoms were entirely gone, but three cases became severe |
